I, the parent or guardian of the above named player,give my approval to participate in the Columbus United Soccer Club 2020/2021 Club Tryouts. I assume all the risk and hazards incidental to such participation and I hereby waive, release, absolve, indemnify and agree to hold harmless Columbus Soccer Organization/Columbus United Soccer Club, the staff and employees, organizers, sponsors, supervisors, participants, contractors, and any persons involved with the activity, for any claim arising out of an injury to my child. Insurance coverage is the sole responsibility of parents or guardian. Columbus United Soccer Club and Columbus Soccer Organization do not carry any type of insurance for participants.
